After ordering you will be e-mailed a patient intake form for our provider to review.  Check your SPAM folder to ensure you receive your confirmation with further instructions.  Please complete this promptly. Medications will not ship until this form is completed and reviewed and an actual prescription has been issued.  Should our provider have any questions they will reach out to you to clarify.  Medications arrive within 2 to 5 business days.
